SIGN UP INSTRUCTIONS
DUTCHESS COUNTY PUBLIC TRANSIT
ADA/FLEX RIDE SERVICE TO NON-EMERGENCY MEDICAL APPOINTMENTS, GROCERY, AND PHARMACY
- Fill out pages 1-3, of application, and sign it.
- There is also a page 4 – which you do not fill out, but must include when submitting your application.
- You may mail your application to:

Bring it to Town Hall 7340 South Broadway – Red Hook, inside an A4 envelope, addressed to the Senior Services Committee – Please leave the envelope in the receptacle outside the front door. The Senior Services Committee Secretary will fax your application to DC Public Transit.
Once the DC Public Transit Office receives your completed application, you may call their hotline: (845)473-8424 Monday – Friday at 9:00 AM to reserve a ride to and from a doctor’s appointment FOR THAT DAY ONLY.
Dutchess Transit will also provide a ride to and from the pharmacy, to pick up medications, and to the grocery store, for food. Call the hotline number at 9:00 AM of the day you wish to go to the pharmacy or grocery store.
